    IMPACT Agewell®How we can help you

    The aim of the IMPACT Agewell ® scheme is to improve the quality of life of older people by providing them with practical services. By working together with your local GP, local Pharmacists and the Northern Health & Social Care Trust, we can help you share relevant information which will support you in managing your long term health conditions and help you access community activities in your area.

    You will be supported by a dedicated IMPACT Agewell ® Project Officer for up to six months who will talk to you about your general health and wellbeing. Together we can support include Befriending, Home Security Checks, Handyperson Services, Energy Efficient Checks, Benefits Advice, Community Transport, Health Condition Support Groups, Luncheon Clubs and Arts & History Clubs.
