The Community Navigator service helps older people in the Northern Health and Social Care Trust (NHSCT) with accessing local services and activities that will improve their health, independence and general wellbeing. Additionally, it supports the NHSCT staff by providing links to the community and voluntary sector. 

The role of the Community Navigator includes taking referrals from individuals, healthcare professionals and the community and voluntary sector for people over the age of 50. They also provide support for community groups so that they can sustain their activities and therefore build capacity through the likes of promotion or information sharing. Most importantly, they raise awareness of older people’s issues by attending relevant forums, meetings, events and consultations 

The Community Navigator can signpost you to various different services, for example a Home Safety Check, a Benefit Entitlement Check, Handyperson Scheme, Good Morning Call, Community Transport, Social Activities, a Luncheon Club and Befriending Schemes.
